Life Care Planning
A Life Care Plan is defined as: "A dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized, concise plan for current and future needs, with associated costs, for individuals who have experienced catastrophic injury or have chronic care needs."
Our Life Care Plan is a dynamic document that will serve to predict the future medical needs of catastrophic cases such as Traumatic Brain Injury, Spinal Cord Injury, Burns and Amputations.
Case management effectively reduces costs of claims, by ensuring prompt medical treatment. Care is directed to the appropriate medical specialists and prompt appointments are coordinated. Case managers communicate directly with the client, the employer and physicians, enhancing consistent treatment. The case manager assists the employer in communicating modified alternative duty availability to the treating provider, thereby reducing lost time.
Rehabilitation Counselors assists individuals who have disabilities with maximizing their potential and their independence. Rehabilitation counselors work with individuals who have a variety of physical, mental and emotional disabilities. They assist individuals in vocational, independent living and educational pursuits.
